On September 29, Advize Health will be partnering with Dr. Phil Smith of MedMorph to deliver a broadcast on Population Health:  Looking at Cause and Effect for Hospital Readmissions and Post-Hospital Mortality.

Chronic disease treatment is exploding in the U.S., and it contributes greatly to our healthcare expenditures, including hospitalizations. While hospital readmissions are decreasing in the U.S., they are still contributing greatly to our national costs and to subsequent penalties to hospitals. For 2015, the Centers for Medicare and Medicaid (CMS) charged 40% of the hospitals with fines totaling $420 million for readmissions and recently announced plans to increase that to $528 million for 2016 with fines to 2,597 hospitals. Yet penalties next year will be on 2011-14 data.  It is difficult for hospitals to avoid penalties on data and outcomes that are 3-5 years old.

This presentation will discuss U.S. costs for chronic diseases, present a cause-and-effect (fish bone) diagram on the contributors to poor post hospital outcomes and discuss strategies to start creating successful performance improvement for hospitals and health systems. You will come away with a better understanding of how chronic diseases are impacting our economy and some strategies to start reducing readmissions and post-hospital mortality.
